Enrollment Information for Mariano Lake Community School
Parents/guardians must provide all requested administrative records before their child can be enrolled at Mariano Lake Community School. Please thoroughly review the registration forms to ensure compliance with all the necessary requirements. For enrollment, students must be accompanied by a parent/guardian.
Requirements for enrollment
All new, current, and transfer student enrollment forms need to be picked up and delivered to Mariano Lake Community School during normal business hours. Online forms are not available.
New and transfer students
Please provide the following documentation for new and transfer students:
- New student enrollment application
- Birth certificate
- Certificate of Indian Blood (CIB)
- Social Security card
- Updated immunization records
- IEP meeting documents, if any
- 4th quarter report cards
- Power of Attorney (Delegation of Parental Rights)
- Withdrawal forms (previous school)
- School transcripts (previous school)
- IEP transcripts (previous school)
Current students
Please provide the following documentation for new and current enrolled students:
- Returning student enrollment application
- Updated immunization records
- Power of Attorney (Delegation of Parental Rights)
